ABSTRACT

Los dientes supernumerarios en la región del tercer molar inferior sonpoco frecuentes. Cuando se presentan en posición distal se les denominadistomolares o distodens. Algunos autores los denominan cuarto molar; ésta denominación es la menos aceptada, ya que el cuarto molarno forma parte de la fórmula dental permanente, sino que es un dientesupernumerario. Cuando éste se presenta paralelo a los molares, en lacara externa o interna de la mandíbula, se denomina paramolar. El manejo quirúrgico suele ser complejo y requiere consideraciones especiales. Presentamos dos casos de dientes supernumerarios en la región del tercer molar: distomolar y paramolar; describimos su diagnóstico, etiología y la técnica quirúrgica empleada.


Supernumerary teeth in the lower third-molar region are rare. When these are located in the region posterior to the third molar, they are known as ®distomolars¼ or ®distodens¼. Some authors refer to them as ®fourth molars¼, a term that is somewhat less accepted due to the fact that the fourth molar is not part of the permanent dentition, but rather a supernumerary tooth. When a distomolar occurs parallel to the molars, either on the outer or inner surface of the jaw, it is known as a ®paramolar¼. Surgical management is often complex and requires special considerations. We report two cases of supernumerary teeth in the third-molar region, one distomolar and one paramolar, and describe their diagnosis, etiology, and the surgical technique employed.

ABSTRACT

En este artículo se describen los supernumerarios, las diversas teorías sobre su etiología, su frecuencia y lasalteraciones que pueden producir. Además, se dan pautas para su tratamiento. Los supernumerarios son anomalías del desarrollo que se encuentran con poca frecuencia, del 0,30 por ciento al 3,80 por ciento de la población, pero en pacientes con labio ypaladar fisurado pueden llegar al 28 por ciento. Se los puede definir como dientes adicionales a la serie dental normal. Se los encuentra raramente en ladentición primaria, en esta la distribución por sexo es similar; mientras que en la dentición permanente el sexo masculino prevalece en una proporción de 2 a 1. El mesiodens es el que se encuentra con mayor frecuencia. Constituye, según Shafer y Cols, más del50 por ciento de todos los supernumerarios...

ABSTRACT

Anomalies of the dentition present real challenges to the dental practitioner. The occurrence of multiple supernumerary teeth in the absence of an associated systemic condition or syndrome is considered as a rare phenomenon. Here, we discuss a case of four supernumerary teeth in one maxillary quadrant with a fusion of supernumerary tooth to maxillary permanent central incisor, which was evident on radiological and clinical examination. Various radiographic views including intraoral periapical radiograph, maxillary occlusal radiograph, orthopantomograph, and cone-beam computed tomographic (CBCT) imaging were done to identify and locate the presence of supernumerary and supplemental teeth. The present case emphasizes the importance of different radiographic views and modalities in correct identification of the dental anomalies and thereby providing a prompt diagnosis and treatment as the exact identification of supernumerary teeth and differentiating it from permanent tooth is of prime importance in treatment planning and management.

ABSTRACT

Dente supranumerário é definido como um distúrbio de desenvolvimento caracterizado pela presença de um ou mais elementos dentários fora do número considerado normal de uma arcada. A ocorrência dos supranumerários pode ocasionar uma variedade de complicações, como por exemplo, apinhamento de dentes permanentes, erupção retardada e/ou ectópica, rotação dentária, formação de diastema, desenvolvimento de lesões císticas, reabsorção de dentes adjacentes. Na maioria das vezes o diagnóstico da presença desse elemento é realizado por exame clínico e radiográfico de rotina. O objetivo do presente trabalho foi relatar um caso clínico de retenção prolongada de um dente decíduo e erupção retardada de um dente permanente em decorrência da presença de um elemento supranumerário diagnosticado apenas aos 12 anos de idade. Após a identificação do elemento supranumerário e análise de exames radiográficos complementares, foi realizada cirurgia para remoção dos elementos decíduo e supranumerário, bem como acompanhamento por três meses do processo eruptivo do elemento impactado, um segundo pré-molar inferior direito o qual se posicionou favoravelmente na arcada dentária. Concluiu-se que a detecção em momento oportuno e a remoção de elementos supranumerários, também respeitadas à necessidade e oportunidade cirúrgica, são importantes para evitar danos futuros à oclusão dos pacientes, devendo-se dar importância ao acompanhamento e evolução clínica do caso...


Supernumerary teeth is defined as a developmental disorder characterized by the presence of one or more dental elements out of a number considered normal arch. The occurrence of supernumerary can cause a variety of complications, such as crowding of the permanent teeth, delayed and / or ectopically erupting, tooth rotation, diastema training, development of cystic lesions, resorption of adjacent teeth. In most cases the diagnosis of the presence of this element is made by clinical examination and routine radiographic. The aim of this study was to report a case of prolonged retention of a primary tooth eruption and delayed a permanent tooth due to the presence of a supernumerary element diagnosed only 12 years old. After identifying the supernumerary element and analysis of complementary radiographs, it was carried out surgery to remove the elements deciduous and supernumerary and monitoring for 3 months of the eruptive process impacted element, a premolar law that favorably-positioned in the dental arch. It was concluded that the timely detection and removal of supernumerary elements also respected the need and surgical opportunity, are important to prevent future damage to the occlusion of patients, should be given importance to the monitoring and clinical outcome...

ABSTRACT

Dentes supranumerários impactados representam um desafio para os Odontopediatras. Estes dentes podem causar atraso na erupção do dente permanente, alteração do crescimento ósseo e posicionamento ectópico do dente permanente. O presente artigo relata a resolução de um caso de dois dentes supra numerários, em ambiente de consultório odontológico, sob sedação e com excelente sucesso clínico radiográfico.


Supernumerary impacted teeth represent a challenge to pediatric dentists. Their presence can cause failure of eruption of permanent teeth, growth and development alterations and displacement of teeth. The case reported in this paper presents the resolution of a case of two supernumerary impacted teeth in a dental office environment, under sedation and with excellent radiographic clinical success.

ABSTRACT

Objetivo: a proposta do presente artigo foi relatar um caso clínico onde, ao serem analisados os detalhes anatômicos na radiografia panorâmica de um paciente previamente ao início do tratamento ortodôntico, não foi detectada a presença de dente extranumerário na região apical do primeiro molar superior esquerdo. Resultado: apesar da presença desse elemento extra, não houve a necessidade de removê-lo para a realização do tratamento ortodôntico, entretanto, poderia em uma outra situação clínica comprometer o bom andamento e sucesso do tratamento. Conclusão: dessa forma, conclui-se com a descrição desse caso clínico que a radiografia panorâmica não substitui o exame periapical no planejamento ortodôntico pois são complementares. Radiografias periapicais apenas dos dentes anteriores poderiam ser insuficientes para o diagnóstico e prevenção dos riscos da terapia ortodôntica.

ABSTRACT

Supernumerary teeth are regarded as an anomalous increase in the number of naturally occurring teeth. Multiple supernumerary teeth can be seen in association with syndromes such as Cleidocranial dysplasia, Gardner’s syndrome, Fabry-Anderson syndrome and Ehlers-Danlos syndrome. Non-syndromic occurrence however is a rarity and very few reported cases exist. Th e exact etiopathogenesis of the condition remains unknown, but several theories have been proposed, the most popular one being “hyperactivity of dental lamina.” When present, supernumeraries often pose a problem to the normal development and eruption of the neighboring dentition. Most of them are accidental fi ndings on radiographs, which also happens to be the most reliable tool in early diagnosis of this abnormality. We describe a case of 28-year-old male having 10 supernumerary teeth with non-syndromic association, which were diagnosed during routine radiographic evaluation.

ABSTRACT

Supernumerary teeth, also known as hyperdontia, is a term used to describe extra teeth that develop in addition to the 20 primary and 32 permanent teeth that we are predisposed to have. Hyperdontia is seen more commonly in the permanent dentition than primary teeth. Th e incidence in the permanent dentition is reported to vary between 0.1% and 3.8%, and its frequency in the primary dentition is found to be 0.3-0.8%. A 32-year-old male patient reported to the department of oral medicine and radiology with the chief complaint of stains and deposits, his clinical examination revealed 9 supernumerary teeth and panoramic radiographic revealed 3 more extra making a total of 12 supernumerary teeth.

ABSTRACT

Presentar un caso de quiste dentígero que invade el seno maxilar asociado a un tercer y cuarto molares ectópicos. Caso clínico: se presenta un caso de quiste dentígero de gran tamaño que invade el seno maxilar en su totalidad, asociado a tercer y cuarto molares, en una paciente de sexo femenino de 14 años. Se realizó la exéresis completa de la lesión mediante un abordaje de Cadwell-Luc. Conclusión: es de suma importancia el diagnóstico temprano de estas patologías a través de un correcto examen clínico y radiológico. En quistes de gran tamaño y de evlución rápida, como es el caso presentado, debemos considerar la posibilidad de una transformación ameloblástica o carcinomatosa en la pared quística. Por esta razón, tendremos que elegir un tratamiento que nos asegurela extirpación total de la lesión y su posterior análisis histopatológico para determinar la conducta por seguir.

Introduction: The talon cusp is defined as a developmental anomaly in which an accessory cusp-like structure projects in the area of the cingulum or cementoenamel junction in the anterior teeth attached to the lingual surface of the crown, ranging in size, shape, length and degree of union with the surface. Case report: This study aimed to report a case of a patient who came to Clinic for Preventive Dentistry of Federal University of Pernambuco, Recife-PE, Brazil, complaining of pain in the upper left region. The clinical exam observed the presence of a supernumerary tooth with talon cusp type III in the canine region which had a carious lesion in the developmental groove at the mesial surface and caused a prolonged retention of permanent tooth. Conclusion: With this we want to emphasize that the Dental Surgeon be aware of the changes caused by dental morphological variations, seeking to conduct a proper treatment plan, meeting the functional and aesthetic needs of the patient.

Los dientes supernumerarios representan una anomalía del desarrollo caracterizada por un excedente en la formula dental normal, su etiología no se encuentra claramente identificada, sin embargo, entre las reportadas, toma importancia la relacionada con alteraciones durante la fase de inicio y proliferación del desarrollo dental. Epidemiológicamente suele presentarse con mayor frecuencia en maxilar que en mandíbula, mas que hombres que en mujeres y con prevalencia en la dentición permanente. El diagnóstico se lleva acabo básicamente mediante radiografías, sin embargo el método clínico puede ser usado de acuerdo las condiciones bajo las cuales se presente. El tratamiento de elección es la excisión quirúrgica. A continuación se reporta un caso de paciente masculino, de 22 años de edad que asiste a consulta para la eliminación quirúrgica de diente supernumerario a nivel de la mandíbula en zona posteroinferior.


Supernumerary teeth are a development anomaly, characterised by over-abundance of teeth in the regular dental formula. The aetiology hasn't been clearly identified, however, among reported cases, alterations during the initial and proliferation phase of dental growth have been found. Epidemiologically, this anomaly appears more frequently in maxilla, male patients and permanent dentition. Diagnosis is carried out by means of radiographies and clinical examination. The treatment of choice is surgical removal. In the following case report, a 22 year- old- male patient who will have supernumerary teeth surgically removed of left posteroinferior mandibular area.

La hiperdoncia es una anomalía dentaria de causa desconocida, caracterizada por el aumento del número de dientes. Ocurre con mayor frecuencia en la maxila, de preferencia en el sexo masculino, siendo más común en la dentición permanente. La presencia de dientes supernumerarios puede ser responsable por alteraciones en la dentición permanente, como por ejemplo, el atraso en la erupción y dislocamientos. Además, el paciente puede presentar oclusión anormal, reabsorción de dientes adyacentes y formación de quistes. El presente artículo describe en un paciente de sexo masculino, de 9 años de edad, la presencia de dos incisivos laterales superiores derechos y un incisivo lateral superior izquierdo, permanentes, parcialmente erupcionados. El examen radiológico reveló un segundo incisivo lateral superior izquierdo, no erupcionado. Se debe resaltar que tanto en la zona de la pieza 1.2 como en la 2.2, los dientes presentaron anatomía normal y no el aspecto conoide comúnmente observado en los dientes supernumerarios. En nuestro caso, con el propósito de efectuar tratamiento de ortodoncia, fue indicado extracción quirúrgica de los dientes mencionados.


The hyperdontia teeth is an anomaly of unknown cause, characterized by increased numbers of teeth. It occurs more frequently in the maxilla, preferably in males, and is more common in permanent dentition. The presence of supernumerary teeth can be responsible for alterations in the permanent dentition, such as the delay in the eruption and dislocations. In addition, the patient may have abnormal occlusion, resorption of adjacent teeth and cyst formation. This article describes in a male patient, 9 years old, the presence of two upper right lateral incisor and one upper left lateral incisor, permanent, partially erupted. Radiological examination revealed a second left upper lateral incisor, unerupted. It should be noted that both, in the region of the 1.2 as in 2.2, the teeth showed normal anatomy and not the conoid appearance commonly observed in supernumerary teeth. In our case, in order to perform orthodontic treatment surgical removal of the teeth noted above was indicated.

Los dientes supernumerarios se refieren a una condición caracterizada por la presencia de un número de dientes superior al normal en los maxilares. El diagnóstico y tratamiento de estas piezas se hace necesario para evitar complicaciones posteriores que pudieran alterar el arco dental. Se presenta el caso de un paciente masculino de 18 años que acude a consulta de cirugía máxilo facial por presentar Síndrome Doloroso Facial, sin causa aparente, al que se le diagnostican 5 dientes supernumerarios retenidos. Los dientes supernumerarios son alteraciones del desarrollo que aparecen en cualquier región de los maxilares, aunque su presencia en la mandíbula es rara. Su etiología no es bien conocida;sin embargo se postula que son el resultado de la “duplicación” de la lámina dental. En estudio de árboles genealógicos se sugiere una posible influencia genética, particularmenteen los mesiodens. Los mesiodens pueden causar varias complicaciones como apiñamiento, retraso en la erupción o desplazamiento de losórganos dentarios. El diagnóstico se realiza de manera clínica y radiográfica.En el presente artículo, se describe el manejo clínico de un caso en un paciente masculino de 4 años 6 meses de edad con tres mesiodens, que acudió a la clínica de la Especialidad de odontopediatría en la Facultad de Odontología, de la Universidad Autónoma del Estado de México.A la exploración intraoral se observó un diente cónico erupcionado, y el análisis de radiografías mostró la presencia de un órgano dentario en la línea media de forma invertida. Debido al desplazamiento observado de los órganos dentarios permanentes, se decidió realizar la extracción del supernumerario cónico erupcionado demanera convencional, y se programó la cirugía del mesiodens invertido. Seis meses después en la radiografía de control previo a la cirugía, se observó la formación de un nuevo mesiodens enel sitio donde fue removido el diente cónico. Se realizó la intervención quirúrgica y se decidió mantener al paciente en observación durante lossiguientes meses.

ABSTRACT

O termo hiperdontia designa os dentes que estão em excesso na arcada dentária. Sabe-se que a arcada dentária normal é constituída de vinte dentes decíduos e de trinta e dois permanentes. Estes dentes podem ser classificados como dentes supranumerários rudimentares, quando não apresentam forma anatômica usual dos dentes presentes na região em que foram encontrados, ou suplementares quando apresentam a forma dos dentes da região. Normalmente são diagnosticados em radiografias de rotina e o tratamento é eminentemente cirúrgico. O presente trabalho tem como objetivo fazer um relato de caso de hiperdontia com oito elementos supranumerários.


The term hyperdontia means teeth that are in excess in the dental arch. It?s known that the normal dental arch consists of twenty teeth and thirty-two permanent. These teeth can be classified as rudimentary supernumerary teeth, if not present the usual anatomical shape of the teeth in the region where they were found, or when additional are in the form of teeth in the region. They are usually diagnosed on routine radiographs and surgical therapy. This work aims to make a case for hyperdontia eight supernumerary elements.

ABSTRACT

A fissura labiopalatal é uma das anomalias congênitas mais frequentes no ser humano, capaz de comprometer o lábio e/ou palato de forma completa ou incompleta. Sua etiologia não é totalmente conhecida, no entanto, é considerada multifatorial, com envolvimento de fatores genéticos e ambientais. Inúmeras alterações sistêmicas decorrentes dessa malformação são encontradas nestes indivíduos. Além disso, a fala, a estética e a posição dos dentes normalmente estão afetados. Sabe-se que uma alta prevalência de anomalias dentárias, como, por exemplo, agenesias, microdontias, dentes supranumerários e hipoplasias são achados comuns na cavidade bucal destes indivíduos. O objetivo deste trabalho é apresentar uma revisão de literatura sobre as anomalias dentárias mais frequentemente observadas em pacientes portadores de fissuras labiopalatais e destacar a importância do diagnóstico precoce realizado pelo cirurgião-dentista no decorrer do tratamento desses pacientes.


Cleft lip and palate is one of the most common congenital anomalies in humans, affecting the lip and/or palate with a cleft completeness or incompleteness. Its etiology is unknown, however, is considered multifactorial, with genetic and environmental components. Many systemic abnormalities arising from this malformation are found in these individuals. Also, speech, esthetics and the position of teeth are usually affected. It is known that a high prevalence of dental anomalies such as agenesis, supernumerary tooth and hypoplasia are common findings in the oral cavity of these individuals. The aim of this article is to present a review of the literature considering the higher prevalence of dental anomalies in patients with oral clefts and highlighted of early diagnosis performed by the dentist during the dental treatment of these patients.

ABSTRACT

The simultaneous occurrence of hypodontia and supernumerary teeth in the same individual is termed 'concomitant hypo-hyperdontia'. There appears to be a correlation between concomitant hypo-hyperdontia and some syndromes, but this anomaly is very rare in the general population. Its etiology is still obscure. Very few cases have been reported in the literature of this condition. ABSTRACT

Hyperdontia is a dental anomaly, very common and widely studied, caused by excessive teeth in the oral cavity. This disturb represents an abnormal formation of one or more teeth in both dental arch. The diagnosis is normally based on conventional radiography exams, mainly panoramic radiographies, and in some cases, a computed tomography. In the present study, 1.325 panoramic radiographies were evaluated, all from patients seen by the Radiology Clinic of Federal University of Ceará, residents in Fortaleza, over the period from October 2007 to December 2008. The aim was to evaluate the occurrence of supernumerary molars. Twenty five patients presented supernumerary molars, 13 men and 12 women. However, it was not observed differences statistically significant (p= 0,5264). The occurrence in the maxilla was more common compared to the mandible. A considerable occurrence of supernumerary molars in the examined population was reported, reinforcing the importance of a premature diagnosis in this dental anomaly, in order to perform the treatment if necessary.


A Hiperdontia é uma anomalia dentária muito comum e largamente estudada, causada por um número excessivo de dentes na cavidade oral. Esse distúrbio representa uma formação anormal de um ou mais dentes em ambos os arcos dentários. O diagnóstico normalmente é baseado em exames radiográficos convencionais, principalmente a radiografia panorâmica, e em alguns casos, a tomografia computadorizada. No presente estudo, 1325 radiografias panorâmicas foram avaliadas, todas de pacientes atendidos na clínica de Radiologia da Universidade Federal do Ceará, residentes em Fortaleza, no período de outubro de 2007 a dezembro de 2008. O objetivo foi avaliar a ocorrência de molares supranumerários. Vinte e cinco pacientes apresentaram molares supranumerários, sendo 13 homens e 12 mulheres. Entretanto, não foi observada nenhuma diferença estatisticamente significante (p= 0,5264). A ocorrência na maxila foi mais comum em comparação à mandíbula. Foi observada uma considerável ocorrência de molares supranumerários na população examinada, reforçando a importância do diagnóstico precoce dessa anomalia dentária, a fim de estabelecer um tratamento, se necessário.

Generalmente, los dientes supernumerarios son asintomáticos, por lo que se diagnostican mediante exámenes radiográficos de rutina. Presentación de un caso clínico en el que mediante un correcto diagnóstico clínico-radiográfico se hallaron evidencias de piezas supernumerarias en ambos arcos dentarios, así como el seguimiento de la situación clínica radiográfica durante más de diez años.
